Checklist & Matrix
To assist you with the proposal process, the following is a checklist of items that must be submitted before the workshop can be approved:
_____ Proposed Offering Form
_____ Instructor’s Application to Teach Form
_____ Instructor’s Syllabus
_____ Instructor’s current vita & official transcripts
_____ If there are additional presenters:
- 75% of presenters must hold a Master's degree or higher
- send a session matrix which must list each presenter, degree earned, title of session instructing, and length of session (sample matrix follows)
