The camp begins Wednesday, November 8th at 9:00 AM and ends on Thursday, November 9th at 4:30 PM.

The Purpose of this camp is to provide education for adults living with Type 2 diabetes. Education will help people with diabetes and their family members gain the knowledge, skills, and attitudes that will promote self-management and a healthier lifestyle. This camp will be offered as a collaboration among regional experts in diabetes education including professionals from Minot State University and Trinity Health.
Who is at Risk? Approximately 90% of people with diabetes are Type 2. This type of diabetes is most often associated with individuals over 40 years of age, however, it is being identified more frequently in younger people. Healthy lifestyles play a very important role in not only the management of diabetes, but also its prevention.
Why Should I Attend?
This camp would allow you to:
- Learn more about diabetes.
- Directly see the relationship between knowledge of the disease and how choices affect your health.
- Assist your spouse or significant other to have a better understanding of the disease process.

Registration Information
A Registration Fee of $39 includes all materials, snacks, and lunches.
Complete the following steps to register for this year’s camp:

2. Print the form and complete the registration.
3. Mail your registration and a $20 deposit (check or money order payable to Minot State University) to:

You may pay the full $39 camp fee with your registration or pay the remaining fee on the first day of camp. We also accept VISA, MasterCard, and Discover. Call (701) 858-3989 or toll free 1-800-777-0750, ext. 3990 for credit card payments.
4. Deadline for registration is Friday, October 27, 2006.
Cancellation and Refund Policy: MSU Center for Extended Learning reserves the right to cancel any activity due to insufficient enrollment. Because the decision to offer an activity is based on enrollment, no refunds are guaranteed if the participant cancels less than three business days prior to the activity. Full refunds are guaranteed if MSU cancels an activity due to low enrollment. All refunds are made by check. Social Security Number is required for refunds.
